The Medicare, Medicaid, and SCHIP Benefits Improvement and Protection Act (BIPA) of 2000 included provisions aimed at improving the Medicare appeals process. Part of these provisions mandate that an independent contractor, known as a Qualified Independent Contractor (QIC), will perform second level appeals known as reconsiderations. QIC contractors render timely and accurate decisions on regular reconsiderations and use a panel review process for medical necessity denials.
First Coast Service Options (FCSO) is a Qualified Independent Contractor (QIC) for Medicare Part B for 34 northern states and three U.S. territories.

For all reconsideration requests, please include all of the following items in your request for the reconsideration:
The Medicare beneficiary’s name
The Medicare beneficiary’s health insurance claim number (“Medicare Card Number”)
Specific service(s) and item(s) for which the reconsideration is being requested, and the specific date(s) each of those item(s) or service(s) were provided
The name and signature of the party (appellant) or the name and signature of the authorized representative of the party (appellant)
The name of the Medicare contractor that rendered the Part B claim redetermination (first-level appeal decision)
